Telephone welfare checks

Dear Practice Managers, Practice Nurses, HCAs and Clinical Directors,

On behalf of the Barnet Training Hub, I have developed a training package designed to support GP practice nurses (PNs) and HCAs to undertake telephone welfare checks on Covid-19 vulnerable patients and, when appropriate, to have advance care planning conversations (ACP) and share that information on Co-ordinate My Care (CMC).

Please find attached a step-by-step guide for how your practices can set this up internally. In addition, we strongly encourage you or your PN/HCA to contact Kyra Rowlatt kyra.rowlatt@nhs.net via email to book a training session. Due to high demand we are able to offer further sessions on Thursday 7th May 10am & 2pm, Thursday 14th May 10am & 2pm and Thursday 21st May 10am & 2pm. The virtual training session lasts 45 mins and is designed specifically to develop skills in ACP for PNs and to provide suggestions including a proposed script for how to have ACP conversations sensitively in the context of the Covid-19 pandemic, alongside a welfare check, and to capture this data for CMC sharing.
